Bay Psychiatry™ of Wisconsin utilizes Valant, a secure, HIPAA-compliant electronic health record (EHR) system, to create, store, and manage your personal health information. Your Privacy Rights

You have the right to:

  • Inspect and obtain a copy of your medical records

  • Request corrections

  • Choose how we send communications (mail, email, secure portal)

  • Limit who can access your PHI

  • See a log of disclosures

  • Get a copy of this notice

  • Appoint someone to act on your behalf

  • File a complaint without fear of retaliation

How We Use & Share Your PHI

We may use or disclose your PHI in the following ways:

  1. Treatment – To coordinate care among your providers.

  2. Payment – For billing and reimbursement, including insurance claims.

  3. Healthcare Operations – For quality improvement and administrative tasks.

We may also share your information:
