Bay Psychiatry of Wisconsin utilizes Valant, a secure, HIPAA-compliant electronic health record (EHR) system, to create, store, and manage your personal health information. You have the right to:

  • Inspect and obtain a copy of your medical records

  • Request corrections

  • Choose how we send communications (mail, email, secure portal)

  • Limit who can access your PHI

  • See a log of disclosures

  • Appoint someone to act on your behalf

  • File a complaint without fear of retaliation
